Your First Visit

We ask that you arrive 10-15 minutes prior to your scheduled appointment to complete our New Patient form. You can also print and fill out the form in advance to bring to your appointment.

New Patient Form

During your first appointment, your dentist will review your medical and dental history, followed by a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening, and any necessary X-rays.

Developing a Treatment Plan

Based on your history and examination, your dentist will develop a treatment plan that best suits your needs. They will happily answer any questions you may have regarding your oral health or treatment options. They will take the time to listen to your concerns and discuss treatment options so you can make informed decisions about your dental care. 

Insurance & Payment Methods

We have developed convenient payment options for you. Prior to proceeding with treatment, fees and financial arrangements will be discussed with you.

At Avery Dental, we'll help you navigate your insurance plan so you can concentrate on achieving your dental goals. If you're not quite sure about the amount of coverage your insurance plan provides, don't hesitate to ask our staff.

Insurance Claims

We will always attempt to bill directly your dental insurance. However, it is understood that the patient is ultimately responsible for fees associated with services rendered. 

Payment Methods

The full treatment cost must be paid at the time of your appointment. We accept most insurance plans and payments by Visa, MasterCard, American Express, debit, and cheques.
